Index card with Emergency Contact Info: regular vet, list of dog's health problems and medications. Also include an authorization form and contact information for someone that will make decisions about dog's care in case you are not able to do so

Photos for ID purposes and a ready-made "lost dog" sign that copies can be made from.

Identification Tags - two sets, one your home and one showing your destination

Food and Water Bowls

Water from home (to avoid upset stomach)

Usual food

Usual treats

Toys and bones

Collar and Leash

Retractable Leash

Flashlight

Pooper scooper

Zip-locked baggies (in case you must travel any distance with refuse, you want an airtight solution)

Pre-moistened towelettes

Paper Towels

Towel

Grooming necessities - nail clippers, brush, blunt-ended scissors, ear-drying solution

Crate

Bedding

Water spray bottle to cool hot dogs.
